A pilot study of bilateral bis monitoring (BBIS) after subarachnoid haemorrhage: will it help to diagnose vasospasm?

Scientific Abstract
We are proposing to carry out a pilot observational study to monitor bilateral BIS recordings throughout the at-risk period for vasospasm (VS) in patients admitted to our unit following aneurysmal subarachnoid haemorrhage (SAH). Our null hypothesis is that global or interhemispheric BIS levels have no correlation with conscious level (GCS) and show no change in the period surrounding new onset neurological deficit. Bilateral BIS strips will be applied to patients on day three following the SAH and left in place for approximately 10 days. We will compare BIS levels in patients who develop clinically significant VS to those who do not. We will particularly be looking at inter-hemispheric changes in BIS that, until the advent of the new bilateral sensors was not possible. We will be looking for changes that may precede VS or occur contemporaneously. Any reversal of these changes following 'triple H' therapy will also be observed.
